Your privacy is very important to me (Miranda Boscawen). This notice is to explain why I might collect your personal data and what is done with it. This is in line with the EU General Data Protection Regulation (GDPR) which came into effect on 25 May 2018; terms from the regulation are indicated in bold.
When you supply your personal details, via the contact form on the A Vital Life Website, when we communicate by telephone, email, text message, what’s app, or you complete a client consultation form or COVID-19 screening and consent form or during a face to face consultation, treatment or meeting, this information is stored and processed in line with the GDPR requirements:
- The need to collect personal information about your health in order to provide you with the best possible service.
- A legitimate interest in collecting that information, to ensure I can practice safely and effectively.
- I keep records of your contact information so that I can contact you in order to confirm your appointments with me or to update you on matters related to your service. This again constitutes a legitimate interest; this is your legitimate interest.
- Provided I have your consent, which includes verbal consent, I may send you information by email or text message in the form of newsletters, offers or articles of interest as well as feedback from a consultation or treatment. You may withdraw this consent at any time – just let me know by e mail Miranda@avitallife.co.uk, or in person during a treatment or telephone conversation 07855 841210 (please don’t leave a voicemail in case I don’t receive it). You can unsubscribe from newsletters via the link on the bottom of them.
I have a professional obligation to retain your records for 7 years after your most recent appointment. You can ask me to delete your records after 7 years if you wish. Otherwise I may retain your records in order to provide you with the best possible care should you see me at a future date.
Paper records are stored in files, in a secure room in my home other than when in transit to see you for an appointment. Where I work in a day clinic setting they are not left overnight in the clinic. Where I see you on a residential retreat I will keep them in a locked portable file until I return home. Electronic records on my laptop or my mobile phone are password protected.
Security patches are applied to my laptop automatically once released by Microsoft.
You have the right to see what personal data I hold on you. You can ask me to correct any factual errors. I am legally required to respond to any request from a client to see their personal data within a timescale of 30 days.
I will never share your information with anyone who does not have a legal right of access without your consent.
I want you to be absolutely confident that I am treating your personal data responsibly. The only people who can access that data have a genuine need to do so. This might be in a situation where I am making a referral with your consent to another health professional or in exceptional circumstances such as risk to your life or the life of others in which case your consent will be sought where possible.
Under a duty to disclose or share your personal data in order to comply with any legal obligation such as relates to Covid-19 I could be asked to disclose to relevant authorities such as the government and NHS information of clients I have seen within a certain time frame.
